(a) Location - 263 Lynn Shore Drive, Lynn, MA: On or about 12/9/15, employees were exposed to falls when working on a pumpjack scaffold platform which extended beyond it's support at least 18 inches.

North Shore Builders Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA Inspection Number 426431 Citation Number 01, Item Number 1,  Issued 5/16/2012 with respect to workplace located at 25-27 Atkins Drive, Lynn MA 01903. The final order date for the citation was on 6/8/2012.
