29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition:   On or about 12/08/2015, at Laird, Inc. located at 3481 Rider Trail South, Earth City, MO, a powered industrial truck (Caterpillar forklift, S/N: ES-24445) was found to be in need of repair, defective, or in any way unsafe, and the truck was not taken out of service until it as restored to a safe operating condition.  This forklift was missing the LPG tank locator/indexing pins and was being operated on the day of the inspection (12/08/2015).

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.:   On or about 12/08/2015, at Laird, Inc. located at 3481 Rider Trail South, Earth City, MO, employees were exposed to hazards such as those created by rotating parts.  The following machines did not have guards:  A JET Equipment & Tools drill press (Model: JDP-20VS-3, S/N: 0090470) located in the Maintenance Shop; and  The Sheldon Machine Co. horizontal lathe (Catalogue No.: ES-56-P, S/N: ES-24445) located in the Maintenance Shop extension.
