29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation.     On or about December 10, 2015, and at times thereto, employees resurfacing pipe were using a flexible cord with a damaged insulation exposing live conductors.  This condition exposed employee(s) to electrical shock hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.  On or about December 10, 2015, and at times thereto, employees resurfacing pipe used a flexible cord where strain relief device was broken.  This condition exposed employee(s) to electrical shock hazards.
