29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. [63 FR 33450, June 18, 1998]:   On or about 12/14/2015, Masterpiece Graphix, LLC located at 1500 Fenpark Drive, Fenton, MO had two separate battery charging stations where employees had to charge and provide some servicing/inspection of the batteries.  There was not an adequate eye wash station available at either charging station.  There was a single bottle eye wash station located approximately 138 feet from the Shipping Departments charger and approximately 108 feet from the Production Departments charger.  There was not an eye wash station which could continuously wash both eyes simultaneously.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years:   On or about 12/14/2015, employees working at Masterpiece Graphix, LLC located at 1500 Fenpark Drive, Fenton, MO, were operating forklifts without having completed the required evaluation/refresher training.  The operators had last attended training on 02/23/2010, but had not completed a 3 year refresher training/evaluation since.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws:   On or about 12/14/2015, Masterpiece Graphix, LLC located at 1500 Fenpark Drive, Fenton, MO, the flexible cord for the Douglas Legacy Power System Ultra Charger in the Production Department was not connected to devices and fittings so that strain relief would be provided.  The strain relief connection would prevent a pull from being directly transmitted to joints or terminal screws within the plug.
