(a)At the plant, an employee was exposed to horizontal intrusion and under-ride hazards while operating standup power industrial trucks in a forks trailing manner without proper guards. The employee was operating a Caterpillar NR4000 Rider Reach Electric forklift when he collided against a lower horizontal crossbar member of a storage-rack shelving system sustained a fatal injury, on or about 12/11/2015.
