29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. On or about December 22, 2015, an employee on the 5:12 pitch roof, operated a pneumatic nailer to attach metal without the use of eye protection, being exposed to struck-by nails and flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about December 22, 2015, the crew supervisor and employees were tearing off and attaching weather barrier paper on the 5:12 pitch roof without the use of any fall protection systems, being exposed to fall hazards from 6-1/2 feet up to 13 feet.
