29 CFR 1918.81(i): Drafts were hoisted when the winch or crane operator could not clearly see the draft itself or the signals of the signalman who was observing the draft's movement.      a. While conducting marine cargo handling operations at Jaxport's Blount Island Marine Terminal, Berth 31, onboard Motor Vessel Citriodora, on or about December 23, 2015, while discharging paper cargo bales, the signalman did not signal to the crane operator that the spreader bar was hung up on the bulkhead of the cargo hold, exposing employees to struck-by hazards from the spreader bar.
