29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  a) Farm, DAF building - On or about 12/17/15 and at times prior thereto a metallic locker cabinet blocked access to a disconnect box mounted on the wall, and in addition, the locker was situated at a 5.0 in. distance from the left side of a 480/277 V dead-front panel which powered the DAF.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Farm - On or about 12/17/15 and at times prior thereto employees performed maintenance/repairs of machinery including (but not limited to) the dissolved air flotation (DAF) machine, compressors, pumps, and peripheral equipment.  The employer did not develop, document, and utilize individual written lockout/tagout procedures.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:      a) Farm - On or about 12/17/15 and at times prior thereto employees conducted maintenance and repairs of machinery including (but not limited to) the dissolved air flotation (DAF) machine and its peripheral equipment which is subjected to lockout/tagout. The employer did not train the authorized employees in relation to a) each equipment and the specific associated lockout/tagout procedure, b) the  types of energies and the specific procedural steps for shutting down, isolating, blocking and securing the equipment to control hazardous energy, c) the specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, and d) the specific requirements for testing the equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
