29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   On or about December 30, 2015, during the installation of roofing material at 1219 Sharon Copley Rd, Wadsworth, Ohio 44281; the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards from working surfaces with a height of approximately 19 feet above the ground.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about December 30, 2015 during the installation of roofing material at the job site located at 1219 Sharon Copley Rd, Wadsworth, Ohio 44281; the employer did not ensure employees working on the ground directly below multiple roofers removing and installing roofing material 19 feet above their location were protected from possible danger of head injury from impact, or from falling or flying objects from the roofing crew working directly above on the roof.
