29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:  On or about February 25, 2019 the employer failed to ensure that employees engaged in commercial roofing in a hoisting area were protected from fall hazards of approximately 40 feet. The employees were receiving and landing roofing material on a low slope roof without the use of a guardrail or personal fall arrest systems.  Chippewa Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1257663, citation number 01, item number 01 and was affirmed as a final order on September 6, 2017, with respect to a workplace located at 245 Main Street, Wadsworth, OH 44281.
