29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  On or about January 7, 2016, at the above addressed jobsite, an employee was exposed to an ejection hazard when operating an aerial lift without a body belt with a lanyard attached to the boom of the aerial lift.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:  On or about January 7, 2016, at the above addressed jobsite an employee engaged in form work operations was exposed to a 10-foot fall hazard when the stairway that he used to gain access to the second level of the building did not have handrails along the unprotected sides.
