29 CFR 1926.850(a): Evidence in writing was not available that an engineering survey performed by a competent person had been performed prior to permitting employee(s) to start demolition operations: a) On or about 01/11/2016 at warehouse building # 3390, two employees were in the work platform of a SJ3 scissor lift #3219, removing an A/C duct system when a large section broke/fell down hitting the lift and knocking it over, exposing employees to a 18 ft. fall hazard. No engineering survey and/or hazardous job analysis was available for the removal of the duct system.
