29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.  On or about January 14, 2016, on the north side of a new residential construction, an employee working beneath an employee installing wood siding while standing on a pump jack was not wearing a hard hat, exposing the employee to the hazard of being struck by falling debris.

On or about January 14, 2016, on the north side of a new residential house, employees installing wood siding to the face of the house were using pneumatic nail guns and did not wear eye protection, exposing themselves to the hazard of being struck in the eye or face by flying debris.
