29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. On or about January 22, 2016, an employee using a Hitachi NT-65M2 pneumatic nail gun was not wearing safety glasses, exposing himself to being hit by nails or flying debris.   b. On or about January 22, 2016, an employee using a Hitachi NR83A3 pneumatic nail gun was not wearing safety glasses, exposing himself to being hit by nails or flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph  1926.501(b):   a. On or about January 22, 2016, on a 6:12 pitch roof, an employee performing siding operations was not protected from a 10-foot fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On or about January 22, 2016, two employees used a 20-foot Werner extension ladder to access a 6:12 pitch roof that was not extended 3 feet over the eave nor was secured from slippage, exposing the employees to a fall hazard.
