29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge:    a- 5th Floor, Back of Building - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to fall hazards, in excess of 10 feet to the roof below, when walking/working next to metal framing that extended beyond its vertical support and was unsupported on its other side.       NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.502(j)(1): Toeboards were not erected along the edge of the overhead walking/working surface for a distance sufficient to protect employees below:    a- 6th Floor Elevator Shaft - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to struck by injuries when the side of the shaft facing the rear of the building and the opening of the elevator shaft were not supplied with toe boards.     b- 5th Floor Elevator Shaft - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to struck by injuries when the side of the shaft facing the street and the other side facing the rear of the building were not supplied with toe boards.      c- Basement - 642 Bedford Avenue, Brooklyn NY: On or about 1/14/16 site workers were exposed to struck by injuries when the side of the shaft facing the street and the opening of the elevator shaft were not supplied with toe boards.
