This violation was most recently observed on or prior to January 14, 2016 in the Tristream Gas Plant located in Eustace, Texas when Process Safety Information pertaining to the piping and instrumentation diagrams (P&ID) was not compiled, complete and accurate, including but not limited to; 1. Drawing 801-102-E58 still call out E-611 A/B (Demethanizer Bottom Cooler) as active process equipment although they were abandoned in place. 2. Drawing 801-102-E54 was not updated for piping 10-CA-0333, 6-P88-B1 and 6-P-8108-B1 routing. 3. Drawing 801-102-E86A was not updated to include 4"-BB-1033 and 6"-BTM-1032 piping; as well as routing for piping 6-P-8108-B1. 4. Drawing 801-102-E87A was not updated to identify 6"-P88-B1 piping. 5. Drawing 801-102-E102 was not updated for routings on Propane Vapor return piping system. 6. Drawing 801-102-E26, unidentified vessel and associated piping was called out on drawing.
