29 CFR 1910.22(a)(1): Walking-working surfaces. General requirements. Surface conditions. All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. a) On or about December 4, 2025, the employer failed to ensure that walking?working surfaces in the chemical transfer area were maintained in a clean and orderly condition. In that hoses were lying on the floor, stacked on top of each other, creating a tripping hazard while employees connected hoses during chemical transfer operations.

29 CFR 1910.36(g)(2): An exit access must be at least 28 inches (71.1 cm) wide at all points. Where there is only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge must be at least equal to the width of the exit access. a) On or about December 04, 2025, in the Drum Line area, the employer failed to maintain an at least 28" inches wide access route when employees were accessing valves located on the opposite side of the drum line. Employees were required to descend from the working platform and move around the conveyor belt to reach the vales, but the available passageways measured 10" inches and 20" inches on the left side and 17" inches and 22" inches on the right side.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following: (i) A list of the hazardous chemicals known to be present using a product identifier that is referenced on the appropriate safety data sheet (the list may be compiled for the workplace as a whole or for individual work areas); and, (ii) The methods the employer will use to inform employees of the hazards of non-routine tasks (for example, the cleaning of reactor vessels), and the hazards associated with chemicals contained in unlabeled pipes in their work areas. a) On or about December 04, 2025, the employer had not created and implement a written hazard communication program for employees working in the chemical transfer area.
