29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two:  a/ Jobsite - Employees engaged in window installation activities working greater than 6 feet from the ground were exposed to fall hazards in that the anchorage point of a horizontal lifeline system was not installed to ensure the safe operation of the fall protection system(s):  1) 29th floor - 30th floor - A synthetic lanyard was looped around a stairway railing while connected to a rope by latch type hook and used as the anchorage point in the horizontal lifeline system.   2) 30th floor - The Sentry Self Retracting (SRL) Lifeline was not installed in accordance with the manufacturer's installation instructions. The SRL was attached to a shock absorbing lanyard that was looped around a stairway railing and used as the anchorage point in the horizontal lifeline system.   Condition observed on or about 1/21/15.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  a Jobsite - a synthetic lanyard that contained incomplete stitching was used as an anchorage point, exposing employees to a fall greater than 6 feet.  Observed on or about January 21, 2016
