29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:  A) Shavings Bin - On or about January 21, 2016, the catwalk, which employees use to access the side of the collection system, was not equipped with intermediate rails near the ladder and was equipped with top rails which were only 38 inches high instead of 42 inches high.  This catwalk was approximately 16 feet above the ground.

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates or markings were not maintained in a legible condition:  A) Worksite - On or about January 21, 2016, a large portion of the metal nameplate, which was located on the left side of the operator's cab, was missing from the Caterpillar R80 forklift, designated as 85-1.
