29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  Job Site - Employees working on a steep sloped roof were not protected from falling.   Shamrock Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 915762, citation number one, item number four and was affirmed as a final order on 8/12/13, with respect to a workplace located at Walnut Street, South Portland, Maine.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Job Site - The ladders used to gain access to the roof did not extend 3' above the roof's eave.   Shamrock Roofing was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 915762, citation number 1, item number 4 and was affirmed as a final order on 8/12/13, with respect to a workplace located at Walnut Street, South Portland, Maine.
