29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions:  a. On or about January 29, 2016, employees working in an excavation 5 feet 11 inches deep were exposed to cave-in hazards at 2204 6th Avenue in Rock Island, Illinois and the employer did not inspect the excavation before the employees were allowed to enter.    b. On or about January 29, 2016, employees working in an excavation up to 6 feet 4 inches deep were exposed to cave-in hazards at 2210 6th Avenue in Rock Island, Illinois and the employer did not inspect the excavation before the employees were allowed to enter.       c. On or about January 29, 2016, employees working in an excavation up to 8 feet 4 inches deep were exposed to cave-in hazards at 2214 6th Avenue in Rock Island, Illinois and the employer did not inspect the excavation before the employees were allowed to enter.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) sloping and benching systems, or paragraph (c) support systems, shield systems, and other protective systems:    a. On or about January 29, 2016, employees working in an excavation 5 feet 11 inches deep were exposed to cave-in hazards at 2204 6th Avenue in Rock Island, Illinois and the employer did not ensure the excavation was sloped, shored, or braced in accordance with the standard.       b. On or about January 29, 2016, employees working in an excavation up to 6 feet 4 inches deep were exposed to cave-in hazards at 2210 6th Avenue in Rock Island, Illinois and the employer did not ensure the excavation was sloped, shored, or braced in accordance with the standard.       c. On or about January 29, 2016, employees working in an excavation up to 8 feet 4 inches deep were exposed to cave-in hazards at 2214 6th Avenue in Rock Island, Illinois and the employer did not ensure the excavation was sloped, shored, or braced in accordance with the standard.
