29 CFR 1926.1101(g)(7)(ii)(A): For all indoor Class II jobs, where the employer had not produced a negative exposure assessment pursuant to 29 CFR 1926.1101(f)(2)(iii), or where during the job, changed conditions indicated there may be exposure above the permissible exposure limit or where the employer did not remove the asbestos containing material in a substantially intact state, the employer did not use critical barriers over all openings to the regulated area:  Building 9361, Supervisor's Office:  On or about February 3, 2016 and at times prior there to, where the employer had not produced a negative exposure assessment, critical barriers were not used. This condition exposed employees to inhalation hazards.

This condition exposed employees to inhalation hazards.

29 CFR 1926.1101(g)(8)(i)(F): Mechanical chipping was performed without a negative pressure enclosure that met the requirements of 29 CFR 1926.1101(g)(5)(i):  Building 9361, Supervisor's Office:  On or about February 3, 2016 and at times prior there to, mechanical chipping was performed to remove asbestos-containing floor tiles using "The Stripper" model number 460/462, in an enclosure that was not under negative pressure.  This condition exposed employees to inhalation hazards.
