29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:       a) In the press room, the employer failed to provide a written hearing conservation program exposing employees to noise hazards. Employee operating manufacturing machines were exposed to an average noise level of 90.1dBA of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 85 dBA) during the 8 hour sampling period.        b) In the paint room, the employer failed to provide a written hearing conservation program exposing employees to noise hazards. Employee operating manufacturing machines were exposed to an average noise level of 87.7dBA of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 85 dBA) during the 8 hour sampling period.

29 CFR 1910.307(c)(2)(i): Equipment in hazardous (classified) location(s) was not approved for the ignitable or combustible properties of the specific gas, vapor, dust, or fiber that was or could be present:  In the paint room, electrical outlet boxes, breaker panels, ventilation motors, and switch lighting in a Class II, Division II location were not intrinsically safe nor approved for Class II, Division II location exposing employee to fire hazards.
