29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts and flying chips:    (a) On or about February 11, 2016 and at times prior to that date, for the employees exposed to caught-in and/or amputation hazards while operating an IKEDA, Type RM 1300, MFG. No. 6833 radial drilling machine located at 311 21st Street NW, West Fargo, North Dakota.      (b) On or about February 11, 2016 and at times prior to that date, for the employees exposed to caught-in and/or amputation hazards while operating a Cincinnati Brickford radial drilling machine located at 311 21st Street NW, West Fargo, North Dakota.                 (c) On or about February 11, 2016 and at times prior to that date, for the employees exposed to amputation hazards while operating a HYDMECH, Model No. S20 Series III, Serial No. 6012141915 metal cutting horizontal band saw with an exposed blade located at 311 21st Street NW, West Fargo, North Dakota.
