29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  (a)Renovations site at 3911 Michael Blvd Front end of the building: On or about February 16, 2016 two employees were observed working/walking on scaffold platform that was not fully decked exposing them to falls of above 10 feet to the ground level.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway shall be at least 18 inches (46 cm) wide.  (a)Renovations site at 3911 Michael Blvd Front end of the building: On or about February 16, 2016, and at times prior thereto, the employer did not ensure that each scaffold platform and walkways used by employees were at least 18 inches wide. Employees used 2X10 wooden planks as scaffold platform.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.   (a)Renovations site at 3911 Michael Blvd Front end of the building: On or about February 16, 2016, and at times prior thereto employee was observed working/walking on scaffold platform which was more than 2 feet above a point of access. The scaffold did not have a ladder to access the platform.
