29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) 49 Preakness Drive, Block 17, Lot 10, Monroe Township, NJ 08831. Two employees were exposed to fall hazards of approximately 22 feet from the eave to the ground below when the portable ladder they were using to access the roof was not secured and extended three feet above the landing.    Condition occurred on or about 02/20/16.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:      a) 49 Preakness Drive, Block 17, Lot 10, Monroe Township, NJ 08831. Two employees were exposed to fall hazards of approximately 22 feet from the eave to the ground below while installing new roofing materials with no fall protection systems in place.       Condition occurred on or about 02/20/16.         Guadzar Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 964547, citation number 1, item number 1a and was affirmed as a final order on 05/06/2014, with respect to a workplace located at Building 1100, Laurel Green, Ark Road, Mt. Laurel NJ.
