29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.  Location - 81 Putnam St. Newton MA,  On or about February 4, 2016, the employer did not ensure that carpenter scaffolds were designed so that the platform did not extend more than 18 inches past the support to protect employees from falling more than 16 feet.
