Employees perform live work and are not aware of the requirements for arc flash protection and what personal protective equipment should be used when performing such tasks.  In addition, employees did not have an understanding of things such as, but not limited to: working on or near de-energized parts' working on or near energized parts; lockout and tagging; re-energizing of equipment or circuits; the appropriate use of personal protective equipment and safe approach distances.
