29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:  (a) Jobsite, 4800 Penn Avenue, Pittsburgh, PA:  On or about 02/29/16 receptacle outlets which were not part of the permanent wiring of the building did not have working ground fault circuit interrupters for personal protection.  The employer had installed the temporary wiring and did not ensure that the ground fault circuit interrupters were operating to provide protection for those persons utilizing receptacles for supplying power to tools and equipment.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  (a) Jobsite, 4800 Penn Avenue, Pittsburgh, PA:  On or about 02/29/16 the employer had not instructed employees who perform electrical work in the recognition and avoidance of shock hazards or what electrical safe work practices could be utilized for the employee's protection.  Employees perform live work and are not aware of the requirements for arc flash protection and what personal protective equipment should be used when performing such tasks.  In addition, employees did not have an understanding of things such as, but not limited to: working on or near de-energized parts' working on or near energized parts; lockout and tagging; re-energizing of equipment or circuits; the appropriate use of personal protective equipment and safe approach distances.
