29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a). On or about March 3, 2016 two employees were working on a 6:12 pitch steep slope commercial roof and were not protected from falling more than 12 feet to the ground below because they were not protected by any means of personal fall protection harnesses, guardrails, or safety nets.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:  a)   On or about March 3, 2016 the employer did not provide a training program to employees who were exposed to fall hazards of 12 feet. Tow employees were working on the commercial steep slope roof without fall protection.
