29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  a). Andrews Visitor and Education Center - On or about March 1, 2016, protective helmets were not worn by employees exposed to an overhead obstruction (i.e. wood trusses) when elevated in a JLG Scissor Lift (model: 2630ES) platform, thus exposing employees to a struck-by hazard.

29 CFR 1926.102(a)(3): The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involve eye hazards, wore eye protection that incorporated the prescription in its design, or wore eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.  a). Andrews Visitor and Education Center - On or about March 1, 2016, an employee wore only prescription glasses while using a DeWalt Circular Saw (model: DWE575SB) to cut plywood, thus exposing the employee to a struck-by hazard.

Andrews Visitor and Education Center - On or about March 1, 2016, an employee worked directly under an elevated Skyjack Scissor Lift (model: SJIII 3226) that had stacked sheets of plywood on top of the platform railing, thus exposing the employee to a struck-by hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  a). Andrews Visitor and Education Center - On or about March 1, 2016, an employee gained access into a Skyjack Scissor Lift (model: SJIII 3226) platform assembly (i.e. basket) by climbing the lift's scissor assembly, thus exposing the employee to fall hazard of approximately 15.6 feet.    b). Andrews Visitor and Education Center - On or about March 1, 2016, employees accessed/egressed the building's roof decking by climbing the railing of a JLG Scissor Lift (model: 2630ES) platform assembly (i.e. basket), thus exposing the employees to a fall hazard of approximately 5.6 feet.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a). Andrews Visitor and Education Center - On or about March 1, 2016, employees that a used Skyjack (model: SJIII 3226) and JLG (model: 2630ES) scissor lift did not receive scissor lift training that includes, but not limited to proper usage, capabilities, limitations, and safety precautions, thus exposing employees to fall and struck-by hazards.
