Andrews Visitor and Education Center - On or about March 1, 2016, protective helmets were not worn by employees exposed to an overhead obstruction (i.e. wood trusses) when elevated in a JLG Scissor Lift (model: 2630ES) platform, thus exposing employees to a struck-by hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. a). Andrews Visitor and Education Center - On or about March 1, 2016, an employee gained access into a Skyjack Scissor Lift (model: SJIII 3226) platform assembly (i.e. basket) by climbing the lift's scissor assembly, thus exposing the employee to fall hazard of approximately 15.6 feet. b). Andrews Visitor and Education Center - On or about March 1, 2016, employees accessed/egressed the building's roof decking by climbing the railing of a JLG Scissor Lift (model: 2630ES) platform assembly (i.e. basket), thus exposing the employees to a fall hazard of approximately 5.6 feet.
