29 CFR 1910.22(c): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc:  a)  On or about March 8, 2016, and at times prior, employees were exposed to fall hazards where the nets in the oil change pits were in disrepair.  The nets installed in the three pits had broken and unraveling netting. (b)  On or about March 8, 2016, and times prior, employees were exposed to injuries from exposed live electrical parts, created by an uncovered opening on an electrical box located near the heater in the basement. 29 CFR 1910.106(e)(9)(iii): Combustible waste material and residues in a building or unit operating area were not kept to a minimum, stored in covered metal receptacles and disposed of daily:  (a)  On or about March 8, 2016, and at times prior, the employer exposed employees to fire hazards in that the metal receptacles, located in the oil pit area, were lined with a plastic bag and contained combustible oily rags which were not covered and/or disposed of on a daily basis.  Additionally, oily rags located in the automotive bay area were kept in a plastic bucket rather than a covered metal receptacle.
