OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing, struck-by and electrical hazards while using automotive lifts:    (a)  On or about February 22, 2018, and at times prior the employer did not ensure that employees         were safe from crushing,  struck-by and electrical hazards due to the following:        (1)  Bend-Pak hydraulic lift power unit, located in bay three, was modified with an electrical switch which was not in       accordance with the manufacturers requirements for safe operation.                  (2) Auto lifts were not periodically inspected by a qualified inspector at least annually to ensure that they were       properly maintained and functioning in a safe manner.    Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
