29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a. On or about March 10, 2016, on the south side of the framed structure, employees installing framing to a new construction residential house while using a pneumatic nail gun did not wear eye protection, exposing the employees to the hazard of being struck in the eye or face by flying debris. b. On or about March 10, 2016, on the north side of the framed structure, an employee installing framing to a new construction residential house while using a pneumatic nail gun did not wear eye protection, exposing the employee to the hazard of being struck in the eye or face by flying debris.

On or about March 10, 2015, on the south side of a 6/12 pitch framed roof of a new residential house, two employees installing framing were not protected from an 11 feet 6 inch fall hazard by the use of a personal fall protection system. b. On or about March 10, 2015, on the north side of a 6/12 pitch framed roof of a new residential house, an employee installing framing was not protected from an 11 feet 6 inch fall hazard by the use of a personal fall protection system.
