On or about, March 11, 2016, employees working alongside forklifts in the packing and warehouse area were exposed to crushing hazards to their feet in that the employer did not provide protective footwear.

29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (l)(4)(ii) when the operator had been involved in an accident or near-miss incident to ensure that the operator had the knowledge and skills needed to operate the powered industrial truck safely.  a.  On or about March 4, 2016, employees were further exposed to the hazards of being struck by forklifts when a forklift operator who had been involved in an accident that caused an employee injury, was allowed to continue forklift operations without receiving refresher training and evaluation for forklift operation.

On or about March 4, 2016, in the wrap area, the forklift was being driven forward with the load obstructing the forward view and path of travel resulting in an injury to an employee struck by the supported load on the forklift.

On or about March 11, 2016, in the receiving and packing area, the BaleMaster vertical baling machine was not guarded in that the gate guard interlock switch was bypassed, exposing employees to an amputation hazard from the shear point of hydraulic ram and front door plate.
