29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function:    a)  ESKO Roofing & Sheet Metal on or about March 21, 2016, at the address of 4505 Lewis Ave., Toledo, OH: The employer failed to ensure a dedicated safety monitor was designated and that his only responsibility was the monitoring function.  The casual use of a monitor created a condition in which employees were performing roofing work at the edge of the elevated surface without the benefit of an active safety monitor. Employees operating in this condition were exposed to an 11 ft. fall to the ground below.
