29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about March 29, 2016, employees were exposed to cave-in/entrapment hazards while working in an excavation approximately 9 foot deep and the employer did not ensure that a ladder of other safe means of egress from the trench was located within 25 feet laterally from the employees.

29 CFR 1926.652(b)(2): Maximum allowable slopes, and allowable configurations for sloping and benching systems, was not determined in accordance with the conditions and requirements set forth in Appendices A and B to this subpart. On or about March 29, 2016, employees were exposed to cave-in/entrapment hazards while working in an excavation and the employer had not ensured that the soil and rock deposit was classified by a competent person and the sloping system used in conjunction with a trench box was configured in accordance with Appendices A and B.
