29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee working from an aerial lift: Worksite located at 2200 Arlington St. Ada, OK 74820. On or about April 07, 2016, and at times prior thereto, an employee was working from the basket of an aerial lift without the use of a fall protection harness and lanyard. The employee was exposed to a fall hazard 20 feet.
