29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).    On or about April 13, 2016, and at times prior thereto, the 25 foot orange extension cord, used to power the Hitachi Disc Grinder, had exposed copper wiring and damage to the internal insulation, exposing employees to the hazard of electrical shock.     The Fox Tank Company was previously cited for a Repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(7)(iv), which was contained in OSHA inspection number 963567, citation number 02, item number 001 and was affirmed as a final order on October 31, 2014, with respect to a workplace located at 117 Airport Commerce Parkway, Kerrville, TX 78028.
