On or about April 21, 2016, employees were not protected from falling to lower level from an unprotected stairwell opening located on the second floor.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. Location - 3-5 Veterans Road, Woburn, MA. On or about April, 21, 2016, three employees were not protected from falling while using a ladder that did not extend at least 3 feet above the upper landing surface. The landing surface was at an approximate height of 24 feet above ground level.
