29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.   Location - 3-5 Veterans Road, Woburn, MA.  On or about April 21, 2016, three employees working/walking on an 8-pitch roof at a height greater than 25 feet above ground level were not protected from falling.

29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with a guardrail system.  Location - 3-5 Veterans Road, Woburn, MA. On or about April 21, 2016, employees were not protected from falling to lower level from an unprotected stairwell opening located on the second floor.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  Location - 3-5 Veterans Road, Woburn, MA. On or about April, 21, 2016, three employees were not protected from falling while using a ladder that did not extend at least 3 feet above the upper landing surface. The landing surface was at an approximate height of 24 feet above ground level.
