On or about April 21, 2016 the employer was using Polyvinyl Chloride (PVC) pipe to supply compressed air with a gauged air pressure of 120 pounds per square inch (psi) to operate portable hand tools throughout the facility. The PVC pipe was not approved for use with compressed air and could fail or rupture, exposing employees to laceration and puncture hazards.
