29 CFR 1910.213(m)(1): Cutting heads of wood shaper(s) were not enclosed with a cage or adjustable guard so designed so as to keep the operator's hand away from the cutting edge:  a) On or about December 8, 2015 an employee was using a Grizzly Brand G9933 multi station wood shaper/ molding cutter that had exposed rotating blades that were not enclosed with a cage or guard so designed to prevent an employee/operator from getting too close to the cutting heads.        1. On December 8, 2015 one employee suffered an amputation injury when his right index finger came in contact with the rotating cutter head.

On May 11, 2016, the employer did not provide OSHA 300 and 301 forms or Workers compensation report forms for one employee who suffered an amputation injury while working in the Carpentry Shop of the employer, with four business hours of the request.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.   a). On or about December 8, 2015 the employer did not record the work related amputation of one employee who suffered an amputation injury to his right index finger when it was caught in the rotating blade of a Grizzly, G9933 multi station router table.   Abatement Certification and Documentation is required
