Site: 421 Hudson Street, New York NY  On or about 4/21/16  a) An employee was fatally struck by a scaffold plank that fell during the tubular welded frame scaffold disassembly. Plank fell from the 9th level of the scaffold.

29 CFR 1926.454(c)(2): Where changes in the types of scaffolds, fall protection, falling object protection, or other equipment present a hazard about which an employee has not been previously trained.  Site: 421 Hudson Street, New York NY  On or 4/21/16  a) Employer did not train the employees in the recognition of hazards associated with falling materials. An employee was fatally struck by a plank that fell from a tubular welded frame scaffold while in the process of disassembling the scaffold.
