OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    a) On the stage of the Lucas Theatre, on or about 04-19-2016, employees working around and under the "Performer Flying System", which was not properly installed and not in safe operating condition, were exposed to being struck-by falling system components resulting in fractures or death.    b) On the stage of the Lucas Theatre, on or about 04-19-2016, the employees using the "Performer Flying System", which was not properly installed and not in safe operating condition, were exposed to falls resulting in fractures or death.
