29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) At Old Olive Street Rd in Creve Coeur, MO 63141 (Multi-Story Apt. Building 1) - On or about 04/26/2016, employees were exposed to elevated fall hazards where a portable job-made ladder was used to access an upper landing surface, 4th floor, and the ladder side rails extended approximately 1 foot above the upper landing surface being accessed.        b) At Old Olive Street Rd in Creve Coeur, MO 63141 (Multi-Story Apt. Building 2) - On or about 04/26/2016, employees were exposed to elevated fall hazards where a portable job-made ladder was used to access an upper landing surface, 2nd floor, and the ladder side rails extended approximately 1 foot above the upper landing surface being accessed.
