29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  At a commercial remodeling construction site, the employer did not ensure that two framing employees using a JLG Lift Model 600S, Serial Number 0300170478 were provided with a safety harness and lanyard exposing them to fall hazards of approximately 24 feet or more to the concrete below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a commercial remodeling construction site, employees were exposed to fall hazards of approximately 24 feet or more to the concrete below without fall protection.
