29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures::      (a) On or about February 15, 2016, an employee was performing work as an outside contractor on a Laempe LFB50--H Sand Core machine while at a host employer's facility located at 1301 Stahl Drive in Warrensburg, Missouri.  The nature of the work mandated the application of multiple lockout / tagout control devices, with occasional releases for trouble shooting purposes.  The employer failed to inform the host employer of their lockout or tagout procedures.
