29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: A) Loading dock area, on or about May 10, 2016, an employee was operating a Yale forklift # 71 that did not have a functioning back-up alarm, and the amber flashing light was missing, exposing employees to a struck-by hazard. B) Loading dock area, on or about May 10, 2016, an employee was operating a Yale forklift # 93 that did not have operable headlights or an operable strobe light, exposing employees to a struck-by hazard. C) Loading dock area, on or about May 11, 2016, an employee was operating a Yale forklift #8 that had a damaged horn, a damaged amber strobe light and a missing brake light, exposing employees to a struck-by hazard.
