29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about May 24, 2016, at the worksite located on North 2nd & Olive Street in Cabot, Arkansas, employees on a canopy applying TPO roofing membrane were exposed to unprotected falls of greater than 6 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about May 24, 2016, at the worksite located on North 2nd & Olive Street in Cabot, Arkansas, the portable extension ladder used to access and descend from the canopy did not extend 3 feet above the landing surface.  Employees were exposed to falls from elevations.
