29 CFR 1904.10(a): The employer did not record all cases on the OSHA 300 Log when an employee's hearing test (audiogram) revealed that the employee had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level is 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS: On or about June 28, 2016, the employer did not record all cases on the OSHA 300 Log when employees' hearing tests (audiograms) revealed that the employees had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears: a) Case No. 18 of the 2016 Plant 1 300 Log was lined out (not recorded) for a Department 71-1 Utility General Assembly employee who experienced a work-related STS revealed during an annual audiogram on March 9, 2016. The case was lined out based on a retest conducted on May 17, 2016, which was not within 30 days of the first test. b) Case No 20 of the 2016 Plant 1 300 Log was lined out (not recorded) for a Department 34-1 Mechanic employee who experienced a work-related STS revealed during an annual audiogram on March 22, 2016.
