On or about June 28, 2016, an employee working at the powder coat transfer line in the south west corner of department 21 was exposed to fall hazards of approximately 69" without adequate guardrails.

At the time of the examination the lift had bent and damaged head lights, a defective horn, defective lights, and defective brakes, none of which were repaired before putting the forklift back into service.

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:    On or about June 3, 2016, the main cover guard and control panel on the American Manufacturing & Technologies, Inc. SLICE S35 grommet cutting machine located in Department 53 of Plant 2 was not affixed to the machine as it only had two of the four bolts in place allowing employees to raise the cover and access the point of operation.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  On or about June 3, 2016, an employee was exposed to the point of operation of the American Manufacturing & Technologies, Inc. SLICE S35 grommet cutting machine located in Department 53 of Plant 2 as the guarding did not prevent the operator from reaching their hands into the danger zone created by the cutter blade and in-feed rolls.
